Counselors or program coordinators may submit STAR enrollment information for schools using the STAR School Form (PDF). Forms must be submitted no later than Sept. 30. You may also submit the information online by clicking here.

STAR student and teacher biographical information may be submitted using the STAR Student/Teacher Bio Form (Word). To ensure that the form prints correctly on two pages: In the print dialogue box, under "Zoom," go to "Scale to Paper Size" and select "Letter." Then print.
